Nephrotic Syndrome

Nephrotic Syndrome Overview:

Nephrotic syndrome is characterised by an increase in permeability of the kidneys due to a loss of podocytes. This allows the passage of protein from the blood to the urine. This leads to increased protein in the urine (proteinuria, and a loss of protein in the blood (hypoalbuminemia).

  • loss of podpcytes
  • proteinuria
    • damage to glomeruli (becomes leaky)
    • frothy urine
  • Dislipidemia
    • liver thinks there isnt enough proteins and cholesterol
  • Peripheral edema
    • hyperproteinuria
    • dc blood vol = activates RAA system and inc retention of H2O and sodium (causing even more fluid to move to the interstitium).

Diagnostic Considerations:

Insert nephritic vs nephrotic syndrome chart once complete.

Therapeutic Aims:

1. Determine And Treat The Cause

  • Diet and Medication Review (NSAIDs, Lithium, Gold, Mercury, antineoplastics, Quinine)

2. Manage Co-morbidities

Especially cardiovascular co-morbiditiesand uremic complications.

3. Reduce Inflammatory Damage In The Nephrons

  • Inhibit TNF-a and NF-kB (Curcuma longa)
  • Flavonoid-rich Herbs (Scutellaria baicalensis, Vitis vinifera)
  • Glucocorticomimetics (Bupleurum falcatum, Rehmannia glutinosa)

4. Reduce Immune Mediated Damage

  • Immunomodulators (Astragalus membranaceus, Echinaceae purpurea, Hemidesmus indicus)
  • ...

5. Tonify Renal Structure & Function

  • Renal Trophorestoratives (Solidago virgaurea, Crataeva nurvala)
  • ...

6. Reduce Oedema

  • Diuretics (Solidago virgaurea, Taraxicum officinale, Asparagus officinalis, Petroselinum crispum)
  • ...

7. Reduce Dysuria

  • Urinary Demulcents (Eltrygia repens, Zea mays, Althaeaofficinalis)
  • ...

8. Prevent & Treat Opportunistic Urinary Tract Infection

  • Urinary Antiseptics (Juniperus communis)
  • Prevent Bacterial Adhesion (Vaccinum macrocarpon)

Differential Diagnosis:

  • Nephritic syndrome
